2. Counter-Notification Process

- Assistance with identification of what content was deleted or to which access was revoked and where the content was located before being deleted or revoked
- A declaration that you have a good faith belief that your content was removed or disabled because of an error or mistaken identification;
- Your name, address, telephone number, and e-mail address
- A declaration that you agree to be under the jurisdiction of the federal district court found at [Your Judicial District], and to accept process of service by the individual who originally made the DMCA notification, or an authorized agent of that individual.
3. Repeat Infringers
Under the DMCA and the available laws, we may, at our sole discretion, terminate, with or without warning, the accounts of users who are determined to be repeat offenders.
